Could it happen again....Too much fuel additive


Mistakes happen and processes are in place across the aircraft world to minimise them. However,this is a serious mistake that shouldn't have been allowed to happen and the first question from me is why did the engineer depart from official documentation for his areas of responsibility?

There's no doubt that the authorities across the board already have this in their sights to prevent further occurrences and I've seen documents that highlight the misuse of kathon.

Furthermore, I have seen the damage that can occur when fuel tanks have been in long term storage with over application of kathon, in turn leading to expensive recovery programs, but in these cases, at least the mistakes were discovered before the aircraft got anywhere near the runway.
